DSF PROGRAM APPLICATION

*THIS IS NOT A REGISTRATION FORM

  1. Please make sure that players meet level requirements

  2. Provide full and accurate information about the player's experience

  3. Once Application is submitted, our DSF Program Coordinator will respond with further instructions

  4. If accepted, our DSF Program Coordinator will send a registration link and a program code to enroll into the DSF Program
